Pathways

Chat + discharge summaries imply multiple templates. One blank medicine list for all patients is wrong.

1. Liver disease (medical) — WhatsApp

• Diet: no-salt + high protein + fluid 1.5 L/day (needs phenotype gates)
• Daily monitor: urine output, BP, pulse, SpO2, sugar

2. DCLD follow-up drugs — WhatsApp (draft)

• Rifagut 550 mg BID
• Looz 30 ml HS
• Pantoprazole 40 mg OD
• Carvedilol 3.125 mg OD
• Multivitamins OD; Calcium + Vitamin D BID

Validate inclusion/exclusion before encoding as defaults.

3. Living donor — sample DS

• High protein; no stairs / heavy lifting / driving × 3 months
• Wound care; Dolo SOS; Pantocid × 2 weeks; Cremaffin; Sucralfate; Becosules; Ultracet SOS
• Review ~5 days with CBC, PT/INR, LFT, KFT, HIDA
• Red flags: fever, altered behavior, pain, headache, not passing urine

4. Post-LDLT recipient — sample DS

• High protein; light activity; no heavy lifting × 6 months
• Immunosuppression: Pangraf (tac), Myfortic, Wysolone
• Prophylaxis: Syscan, Bactrim DS
• Pantocid, vitamins/Ca/D, Magnorate, Cremaffin, Dytor
• Glucose sliding scale if needed; tac level timing; scheduled labs
